EMS Learner Declaration

• I confirm that I am enrolling as an EMS learner.

• I declare that to the best of my knowledge all the information supplied on, and with, this form is true and complete

• By submitting this form, I confirm that I will abide to agree by EMS’:
            o enrollment terms and conditions
            o  attendance, minimum learning hours and self-study required
            o  learner code of conduct
            o confidentiality of the EMS’ intellectual properties and copy rights
            o academic honesty standard
            o health and safety policy
            o pastoral care for learners
            o learner feedback and complaint procedure
            o If applies, withdrawal and refund policy
            o other student acknowledgment and declaration set out

• I understand that if the information provided by me is subsequently found to be false or inaccurate, my enrollment may be cancelled, and I agree that I will be liable for any costs incurred by EMS in cancelling my enrollment.

• I consent to the disclosure of personal information as required by the relevant government agencies.

• I agree to the use and disclosure of my information to my parent or student-appointed representative or relevant government agencies for any purpose related to my education or well-being both before and after admission. 

• Throughout my study, I agree not to pursue my right on professional indemnity claim

• I agree that I am responsible for my own books, equipment,  and personal items and I release EMS from all liability  and claims for loss or damage to such terms, howsoever caused.

• I understand that I may be invited to attend activities organised by EMS as part of my course. Before th activities, I will be notified if there are any fees involved.

• I authorize EMS to obtain relevant professional services, including health, mental, medical treatment, or other pastoral care services for me should such action be deemed necessary by EMS or a staff member acting on behalf of EMS.

• I agree to indemnify EMS for any expense, loss, damage, or liability of whatsoever nature as a result of authorizing and arranging such emergency medical treatment.

• I agree to protect EMS’ reputation wherever applicable.
